Stride Academy collects personal health information about your child and family during assessment and treatment. Some examples of the personal health information we collect may include: name, date of birth, address, phone number, records of sessions your child progress during those sessions. Your personal health information will be contained within our client files, which are locked in a cabinet located in a locked room. We limit collection of information only to what is required to provide assessment and treatment to your child. We have safeguards in place to make sure your information stays private. Only people who need to access your personal information are allowed to access it. We have procedures in place to monitor and manage our legal responsibility to protect personal health information. We take steps to ensure that all staff protects your privacy and only use your personal health information for the intended purposes you have consented to.
Uses and disclosures of personal health information
We use and disclose personal health information to:
- Plan, deliver, and coordinate services for children and families.
- Conduct risk management and quality improvement activities.
- Comply with legal and regulatory requirements.
Parents/caregivers may limit access, make corrections to their children’s personal health records, ask to see and get a copy of your health record, or withdraw consent for some of the above uses and disclosures by speaking with a staff person or emailing us at admin@strideacademy.ca.
Consent
We require that the individual, family, or caregiver give consent to collect personal information when accessing our services. We also obtain consent for us to share your information with our staff involved in assessment and treatment. Finally get consent to share your information with other people and to obtain information from other providers if it is necessary for assessment and treatment.
There are times when we must share information, with or without your consent. Those times include when we believe a child has been at risk, or is at risk of being harmed, where there is intent to harm oneself or someone else, or when our staff are required to release information to the courts.
